Let’s Build a Long-Term Relationship

We look forward to serving your workers’ compensation needs and developing a long-term business relationship with brokers like you. To get started, please complete our Brokerage Agreement. It sets forth the terms and conditions by which you and Agency Resources agree to do business.

Provide the following documentation via fax to 866-943-4039 or submit via email:

  1. A fully completed and signed Brokerage Agreement
  2. A fully completed and signed W-9
  3. A copy of your current and valid insurance license
  4. Proof of E&0 coverage

Agency Resources must have all documentation on file before we can bind coverage.

Note: You may submit your first request for coverage with your completed Brokerage Agreement and documentation.
